Abstract

The Trust-Centric Enterprise System Architecture (TESA) provides a structured framework for healthcare-financial integration that prioritizes transparency, user agency, and ethical governance while addressing growing public concerns about data stewardship that have created trust deficits limiting service adoption and perpetuating inequities. TESA addresses these challenges through four interconnected layers—data sovereignty, algorithmic accountability, inclusive interfaces, and community governance—which systematically embed trust-building mechanisms within enterprise architecture, drawing from institutional trust theory, distributed governance models, and fairness frameworks to enhance user engagement, impartiality, and system stability across diverse socio-economic contexts as demonstrated in implementations in Estonia, India, and other regions. Through these mechanisms, TESA enables organizations to not only meet regulatory compliance requirements but to fundamentally transform their relationships with users, establishing durable trust foundations that transcend traditional security measures and privacy protections. By emphasizing user empowerment and ethical data stewardship, the framework offers a viable path toward healthcare-financial systems that achieve both operational excellence and social legitimacy in an era of increasing digital skepticism. Despite challenges related to efficiency trade-offs, economic sustainability, power imbalances, legacy system integration, and regulatory uncertainty, TESA represents significant progress toward digital infrastructure that equitably serves diverse populations while rebuilding essential trust in critical services.
